Почти у 60% детей нарушение мозгового кровообращения (НМК) манифестирует с острых симптоматических эпилептических приступов, у 30% пациентов в отдалённом периоде инсульта развивается постинсультная эпилепсия (ПИЭ). Отсутствие единой терминологии и чётких временных критериев как острых симптоматических эпилептических приступов, так и ПИЭ, усложняет анализ исследуемых групп пациентов, перенёсших НМК. Множество наблюдений ограничены маленькими группами, использованием собственной терминологии, различными возрастными медианами и критериями включения. Как клинические, так и инструментальные факторы риска развития ПИЭ в детском возрасте не определены, что затрудняет выявление групп риска по развитию эпилепсии и возможности прогнозирования исхода в отдалённом периоде. Единственным значимым фактором риска в большинстве публикаций выделяется «младший возраст», однако его границы определены нечётко. Большинство исследований являются ретроспективными, что может влиять на результаты встречаемости и определение типа эпилептических приступов при остром НМК. Электроэнцефалография — наиболее информативный метод выявления субклинических изменений и выделения групп риска. Продолженная электроэнцефалография эффективна, особенно в выявлении бессудорожного эпилептического статуса. В настоящее время не существует единых протоколов обследования пациентов с острым НМК как в дебюте, так и в катамнезе. The lack of a unified terminology and temporal criteria for both acute symptomatic epileptic seizures and post-stroke epilepsy complicates the analysis of the studied groups. Many studies are limited to small groups, proprietary terminology, different age medians, and inclusion criteria. Both clinical and instrumental risk factors for the development of post-stroke epilepsy in childhood have not been identified, which makes it difficult to identify risk groups and predict the outcome in the long term period. The only significant risk factor in most publications is «younger age», but its boundaries are blurred. Most studies are retrospective, which may affect the incidence and type of epileptic seizures in acute cerebrovascular accident. Electroencephalography is the most informative method for detecting subclinical changes and identifying risk groups. Continuous electroencephalography is effective primarily in the detection of non-convulsive status epilepticus. Currently, there are no unified protocols for examining stroke patients, both at the onset and in follow-up.
