Для лучшего исхода противоэпилептического вмешательства крайне важна точная визуализация структурного эпилептогенного очага на МРТ. Помимо идентификации эпилептогенного субстрата МРТ часто помогает уточнить этиологию эпилепсии.</p><p>Цель работы — уточнить природу эпилептогенного очага у детей с фармакорезистентной эпилепсией, проходивших комплексное прехирургическое обследование и оперативное лечение в Российской детской клинической больнице, и сопоставить диагноз пред­операционного МРТ с результатами патоморфологического обследования.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен анализ данных нейровизуализации, клинико-неврологических, нейропсихологических, электрофизиологических исследований 65 детей, обследованных в рамках прехирургического комплекса в отделении лучевой диагностики и оперированных в отделении нейрохирургии в 2016–2020 гг.</p></sec><sec><title>Результаты</title><p>Результаты. У детей с фокальной эпилепсией, перенесших противоэпилептическое хирургическое вмешательство, выявлены разнообразные по этиологии и гистологической природе эпилептогенные структурные изменения мозга: кортикальные дисплазии (44,6%), глиоз и рубцово-атрофические изменения паренхимы мозга (27,7%), опухоли (7,7%), энцефалит Расмуссена (10,8%), сосудистые мальформации кортикальной локализации (4,6%), нейрокожные синдромы (3%) и другие редкие варианты эпилептогенных поражений (1,5%). Фокальные кортикальные дисплазии оказались часто выявляемым структурным поражением у детей с фармакорезистентной фокальной эпилепсией — 33,8%. Эпилепсия характеризовалась прогредиентным течением, тяжелыми частыми приступами, усугублением психомоторного и адаптивного статусов. </p></sec><sec><title>Заключение</title><p>Заключение. Специализированный протокол МРТ высокого разрешения и принцип мультидисциплинарного подхода в хирургическом лечении эпилепсии, реализованный в РДКБ, позволяет с высокой степенью точности локализовать эпилептогенный очаг и предположить гистологическую природу поражения головного мозга, что подтверждает частое (95,2%) соответствие диагноза предоперационной МРТ и патоморфологического исследования.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. MRI revealing the epileptogenic foci is the central part of the presurgical evaluation of children with focal-resistant epilepsy. Besides that, MRI seems to be a reliable instrument of treatment optimization and good prognosis.</p></sec><sec><title>Aim</title><p>Aim. To identify epileptogenic lesions in children with resistant focal epilepsy by high resolution (HR) MRI and comparing obtained data with postsurgical morphology.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We assessed the results of neuroimaging, EEG, clinical neurological status, and postsurgical morphological data in 65 children with focal epilepsy who had undergone antiepileptic surgery in Russian State Children Hospital, neurosurgery department (Moscow, Russia) from 2016 to 2020. </p></sec><sec><title>Results</title><p>Results. The broad spectrum of epileptogenic lesions was revealed by HR MRI, including focal and diffuse cortical dysplasias (44.6%), cortical post hypoxic gliosis (27.7%), glial tumors (7.7%), Rasmussen encephalitis (10.8%), tuberous sclerosis, and Sturge–Weber syndrome (3%), small angiodysplasias (4.6%) and other. Focal cerebral dysplasia had an obvious superiority as the main etiological factor in focal epilepsy and was approved in 33.8% of all patients. In all cases, epilepsy is characterized by frequent and resistant seizures, inflicting motor arrest, and psycho-emotional deterioration. </p></sec><sec><title>Conclusion</title><p>Conclusion. HR MRI and multidisciplinary investigation have to be unchangeable standards in the presurgical evaluation of children with focal epilepsy.
