В мире мигренью страдает более миллиарда человек. Одной из наиболее тяжёлых форм данного заболевания является хроническая мигрень, при которой приступы головной боли возникают 15 и более дней в месяц (из них не менее 8 — типичные приступы мигрени). Согласно действующим клиническим рекомендациям для лечения хронической мигрени с 2010 г. применяется ботулотоксин А. Рандомизированные мультицентровые плацебо-контролируемые клинические исследования проводились к настоящему времени только для онаботулотоксина А, однако появились данные о возможном применении с этой целью инкоботулотоксина А. Он обладает рядом преимуществ (меньший риск образования антител, хранение при комнатной температуре, более экономичен), поэтому возможность его применения для лечения хронической мигрени представляет несомненный исследовательский и клинический интерес.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Мы провели ретроспективное изучение амбулаторных карт 27 пациентов в возрасте 23–55 лет с хронической мигренью, получавших 4 курса инъекций 145 ЕД инкоботулотоксина А подкожно в 31 точку каждые 12 нед. Результаты лечения оценивали после 1-го и 4-го курсов инъекций препарата.</p></sec><sec><title>Результаты</title><p>Результаты. Снижение количества дней с мигренью после 1-го курса инъекций инкоботулотоксина А на 50% и более отмечено у 74,07% пациентов, после 4-го эффект сохранялся у 73,07%. Снижение количества дней с головной болью после 1-го и 4-го курсов на 50% и более отмечалось у 48,14 и 50% пациентов соответственно. Интенсивность боли по визуально-аналоговой шкале в оставшиеся дни снизилась на 41,3%. Отмечено количественное уменьшение приёма лекарственных препаратов в среднем на 67,2 ± 2,1%. У пациентов с положительным эффектом на терапию отмечалось снижение уровня тревоги и депрессии по Госпитальной шкале тревоги и депрессии. У всех пациентов, ответивших на терапию, сохранялся стабильный эффект в процессе ботулинотерапии. Лечение характеризовалось хорошей переносимостью, низкой частотой и незначительной выраженностью нежелательных эффектов.</p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования. Данное исследование имеет некоторые ограничения. Характеристики некоторых пациентов неоднородны (возраст, стаж мигрени), а размер выборки ограничен. Проведённый ретроспективный анализ на базе одного амбулаторного центра может ограничить применимость и обобщение полученных результатов. Кроме того, данное исследование осуществляли в условиях реальной клинической практики, поэтому оно не включает контрольную группу пациентов, получавших плацебо.</p></sec><sec><title>Заключение</title><p>Заключение. More than a billion people suffer from migraines worldwide. One of the most severe forms of this disease is chronic migraine, in which headache attacks occur 15 or more days a month (at least 8 of them are typical migraine attacks). According to the current clinical guidelines for the treatment of chronic hemicrania botulinum toxin A is used. Randomized multicenter placebo-controlled clinical trials have been conducted to date only for onabotulotoxin A. However, data on possible use of incobotulinumtoxin A for this purpose has appeared. It has a number of advantages. For example, it has lower risk of antibody formation, is stored at room temperature and more economical. So, the opportunity of its use for the treatment of chronic hemicrania has undoubted research and clinical interest.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We conducted a retrospective study of outpatient records of twenty seven 23–25 years patients with chronic hemicraniawho received 4 courses of injections of incobotulinumtoxin A (Xeomin, «Merz», Germany) with 145 units subcutaneously at 31 point every 12 weeks. The results of treatment were evaluated after the 1st and 4th courses of injections of the drug.</p></sec><sec><title>Results</title><p>Results. After the 1st course of injections of incobotulinumtoxin A the days number with hemicrania decreased by more than 50% in 74.07% of patients, after the 4th course the effect remained in 73.07%. After the 1st and the 4th course of injections a number of days with headache decreased by more than 50% in 48.14% and 50% of patients, respectively. Intensity of pain on the visual-analog scale (VAS) decreased by 41.3% in the remaining days. There was also a quantitative decrease of drugs intake on average by 67.2 ± 2.1%. There was a decline in the anxiety and depression level according to the Hospital Anxiety and Depression Scale (HADS) in patients with a positive treatment effect. All patients responded to therapy maintained a stable effect during botulinum therapy. The treatment was characterized by good tolerability, low frequency and insignificant severity of undesirable effects.</p></sec><sec><title>Limitations</title><p>Limitations. This study has some limitations. The characteristics (age, hemicranias) in some patients appared to be inhomogeneous, and the sample is limited. It was also a retrospective analysis on the basis of one outpatient center, which may limit the applicability and generalization of the results obtained. In addition, this study was carried out in a conditions of real clinical practice, therefore, the study does not include a control group.</p></sec><sec><title>Conclusion</title><p>Conclusion.
